Screening & Intake Assessment
A face to face screening and intake process is completed within 14 days of the initial visit. The strengths, needs, abilities, and preferences of each recipient are identified explicitly for integration within an individual plan of care.
Individual, Group, and Family Therapy
Each recipient is assigned a licensed therapist who is responsible for the recipient’s coordination of care. Individualized, group, and/or family therapy sessions take place to assist the recipient in both individual treatment plan goals and objectives.
Progress Reviews and Individual Treatment Plan Reformulation
Regular scheduled meetings are held with each recipient to review the progress of their individual goals and update and/or revise the Individual Treatment Plan.
Case management
refers to a collaborative process that helps individuals with mental health or substance use disorders access and coordinate the services and resources they need to achieve recovery and improved quality of life.
Psychoeducation
refers to the process of educating individuals—and often their families—about mental health conditions, treatment options, coping strategies, and available resources to help them better understand and manage their symptoms.
Medication training and support
refers to the education and assistance provided to individuals to help them understand, manage, and adhere to prescribed psychiatric medications as part of their overall treatment plan.
Comprehensive Individual Treatment Planning
An individual treatment plan is developed with each recipient within the first 14 days of treatment. The recipient is a full participant in the process and goals and objectives are based on individual needs, strengths, abilities and preferences. Goals are stated in the recipient’s own language, and the recipient has open access to the individual treatment plan upon request.
Psychosocial Education
Psychosocial education is available to recipients, families, care providers in both individual and group formats and provided to assist with interpersonal relations, role performance, anger management, and communication skills.
Discharge Planning and Criteria
Discharge planning occurs throughout all phases of the program. The person served has met discharge criteria when the goals of the individual plan are achieved and referral to support services is completed, when appropriate.
Crisis intervention
is a short-term, immediate response aimed at assisting individuals experiencing an acute psychological, emotional, or behavioral crisis. The goal is to stabilize the person, reduce symptoms of distress, and restore them to their baseline level of functioning or connect them with longer-term support.
Skills Training
is a therapeutic approach focused on teaching individuals practical skills to manage mental health symptoms, improve functioning, and enhance overall well-being.
Psychosocial rehabilitation (PSR)
is a person-centered, strengths-based approach that helps individuals with serious mental health conditions develop the skills, supports, and confidence needed to live independently and participate fully in their communities.
